Athletics Forms due by July 16

Last month, the Athletics Office sent to all new and returning Upper School families an envelope containing the Interscholastic High School Sports Participation Form, CIF Athletic Agreement and Code of Conduct, and the 2011 Summer Practice Schedule. Parents should read the enclosed letter and return the signed forms in the gray return envelope provided no later than July 16. The forms and summer schedule also can be downloaded on the Poly athletics webpage (see "Downloads" on the right column on the page).
