Spring Health Office Reminders

Our new health forms platform, August Schools, is now live and ready for your students' health updates for the 2025-2026 school year! Please complete all forms within the August Schools health enrollment packet, including immunization records, medication and allergy information, and a brief questionnaire about each student's health history. All health enrollment forms must be submitted by July 15, 2025.

The Poly Athletic Department and the Health Office have partnered with Exer Urgent Care to provide complimentary sports physicals for Poly students from May 1 to August 31, 2025. We strongly encourage you to take advantage of this complimentary service as soon as possible to ensure all necessary paperwork is completed on time. Book your appointment here.
