Important: Deadline for health forms was July 15

Just a reminder that the July 15th deadline for completing online student health forms for the 2018-19 school year has now passed. In addition, any new Upper School student participating in a summer athletic team practice must have submitted his or her student health records prior to the start of practice.

In addition, please note:

• Poly requires all Middle and Upper School students to have an annual physical even if the pediatrician does not require a yearly physical. Physicals must have taken place after Jan. 1, 2018. The Physician Consent Form is available online through Magnus.
• The Tdap vaccine is required for all seventh-grade students. The vaccine must have been given on or after the student's 7th birthday, and the vaccine record must be included in the student's immunization record in Magnus Health prior to the start of seventh grade.

To access your student’s online health record:

• Log in to MyPoly.
• Click on the Magnus Health button on the Resources page.
• Watch the pop-up online video tutorial.
• Complete all of the required fields and submit all required forms via upload, fax, or mail directly to Magnus.

If you have never logged into the Poly website, visit MyPoly and click the "Need help logging in?" button to retrieve your password. When requesting your password, you will need to use the email address through which you receive Poly communications as the email on record. Should you experience any difficulty logging in, feel free to contact the Communications Department.
