Athletic forms due last week

The Athletics Office recently sent to all new and returning Upper School families an envelope containing the Interscholastic High School Sports Participation Form, CIF Athletic Agreement and Code of Conduct, CIF Concussion Sheet, and the 2012 Summer Practice Schedule. Parents should have read the letter enclosed in the packet and returned the signed forms in the gray return envelope provided by June 15. The forms and summer schedule also can be downloaded on the Poly athletics webpage (see "Downloads" on the right column on the page). Questions should be directed to Athletic Director Steve Beerman.
