Reminder to complete online health forms by July 15

In addition, please note:

• Poly requires all Middle and Upper School students to have an annual physical even if the pediatrician does not require a yearly physical. Lower School students are not required by the school to have an annual physical until sixth grade. The Physician Consent Form is available online.
• Middle and Upper School parents should schedule their child’s annual school physical soon—appointments can take weeks, even months, to schedule. Physicals must have taken place after Jan. 1, 2014.

To access your student’s online health record:

• Log in to MyPoly.
• Click on the Magnus Health button.
• Watch the pop-up online video tutorial.
• Complete all of the required fields and submit all required forms via fax or email as indicated.

If you have never logged into the Poly website, visit MyPoly and click the "Need help logging in?" button to retrieve your password. When requesting your password, you will need to use the email address through which you receive Poly communications as the email on record. Should you experience any difficulty logging in, feel free to contact the Poly Communications Department.
